School Nutrition During School Closure

MELA will provide meals for all children who need them, no matter their resources, and no matter what school they attend.

If you require MELA to provide School Meals during the School Closure per Governor Waltz Executive Order regarding the Coronavirus COVID-19 Pandemic, please complete this form for each school-age child in your household.

Updates to this form will be regularly viewed so we can make appropriate arrangements for deliveries.
